Home Health

Home Health represents a wide range of medical and support services provided in the home. Your Pacific Point Health Care Services professional team will monitor your progress and keep in contact with your physician to make sure that your treatment goals are met. Services provided may include: post-surgical management, chronic illness management, wound care, catheter care, pain management and symptom control, physical therapy, occupational therapy, speech therapy and nutritional counseling.


Skilled RNs perform patient evaluations, including monitoring and tracking vital signs, clinical procedures such as IV infusion therapy, phlebotomy, and administering medications.

In the home, the nurse will monitor your progress and keep in contact with your physician to ensure treatment protocols are appropriate.

Speech Pathologists are responsible for the assessment and treatment of speech, language, and swallowing, cognitive and voice disorders. Through the use of multiple therapy techniques, therapists provide a well-rounded program of care to help remedy a speech or swallowing problem.

The palliative care physician works with the patient’s primary doctor. The palliative care physician closely monitors the progression of illness and prescribes medications and healthcare directions to other members of the team.

The interdisciplinary team coordinates and supervises all care 7 days a week, 24 hours a day. This team is responsible for making sure that all involved disciplines share information. This is achieved through care staff meetings and family care conferences.
